Dan Buettner's Potluck Moai Launch

On July 8, join Blue Zones Project® for a 12-week Plant-Based Potluck Moai Launch with special guest, Dan Buettner.

Participants agree to the following:

· Meet with your Moai group at least 5 times between July 15 – Sept 6 to share plant-based dishes

· Practice adding plant-based dishes to your daily diet

· Take the online True Vitality Test (no cost) before starting Moai and again at the end of the 12 weeks. This will be emailed to you before the Moai Launch event on July 8.

· Record weight, blood pressure and available labs (blood sugar or A1c; cholesterol, c-reactive protein) before starting Moai and record again at the end of the 12 weeks

· Be available to the NBC Today Show for possible periodic interviews about your Potluck Moai experience

**By registering, you agree to commit to the full 12 weeks of the Plant-Based Potluck Moai. To participate, attendance is mandatory at the July 8 Moai Launch.
