NO PURCHASE OR ACTIVITY OBLIGATION IS NECESSARY TO ENTER OR WIN. SWEEPSTAKES SHALL BE CONSTRUED AND INTERPRETED UNDER THE LAWS OF THE UNITED STATES AND SUBJECT TO ALL APPLICABLE FEDERAL LAWS AND REGULATIONS. VOID WHERE RESTRICTED OR PROHIBITED BY LAW.

I. ELIGIBILITY: OPEN ONLY TO RESIDENTS OF PARTICIPATING BLUE ZONES PROJECT COMMUNITIES (“COMMUNITY(IES)”) WHO ARE LEGAL RESIDENTS OF THE FIFTY (50) UNITED STATES AND THE DISTRICT OF COLUMBIA (“D.C”) WHO ARE AT LEAST EIGHTEEN YEARS OF AGE (OR AGE OF MAJORITY IN JURISDICTION OF RESIDENCE, WHICHEVER IS GREATER).

Entries received from persons residing outside of the eligible geographic Communities, or where restricted or prohibited by law, or persons who are not legal citizens of the United States, will be disqualified. Participation constitutes each person’s full and unconditional acceptance of these Official Rules. Eligibility to receive a prize is contingent upon an individual fulfilling all requirements as set forth in these Official Rules. Officers, directors and employees of Blue Zones Project by Sharecare, Inc. (the “Sponsor” or “BZP”) and their parent companies, affiliates, subsidiaries, advertising, promotion, fulfillment or other coordinating agencies, individuals providing services to Sponsor through outsourcer (collectively “Released Entities”), during the Promotion Entry Period (defined below in Section II), and their respective immediate family members and persons living in their same household, are not eligible to participate in the Promotion. For purposes of this Promotion, immediate family members are defined as spouse, partner, parents, legal guardians, in-laws, grandparents, siblings, children and grandchildren and those living in the same household shall mean people who share the same residence at least three (3) months a year, whether legally related or not. Sponsor reserves the right to verify the eligibility of each entrant and to disqualify any entrant that Sponsor deems does not meet the eligibility requirements. All federal, state and local laws and regulations apply. Void where prohibited.
 
II. SWEEPSTAKES ENTRY AND QUARTERLY ENTRY PERIODS: The
Live Longer, Better Rewards Program (“Program” “Promotion” or “Sweepstakes”) begins January 1, 2022 at 12:01 a.m. Eastern Time (“ET”) and ends on December 31, 2022 at 11:59:59 p.m. ET (the “Entry Period”) which is made up of four Entry Periods (“Quarter Entry Period”): Quarter I (1/1/22 to 3/31/22); Quarter II (4/1/22 to 6/30/22); Quarter III (7/1/22 to 9/30/22); Quarter IV (10/1/22 to 12/31/22).

 

III. HOW TO ENTER THE PROGRAM AND SWEEPSTAKES: A resident of a participating Community will be deemed a Participant (“Participant” or “you”) in the Sweepstakes if eligible and enters the Sweepstakes using at least one (1) of the two (2) following methods of entry:


1.) Activities and Events Methods of Entry:
Eligible Participants are required to have an active e-mail account and Internet access, which is used to invite Community residents to register and participate in the Blue Zones Project Live Longer, Better Rewards Program (the “LLBR Program”).

a.) Create an Account: In your Community’s e-mail invite, click on the link that takes you to your Community’s registration page to create an Account (“Account”). You are requested to complete the optional RealAge Test. Then follow the prompts to view the LLBR Program elements and begin participating in the LLBR Program’s quarterly Activities (“Activity(ies)”) and one (1) Community Event (“Community Event” or “Event”).

 

b.) Members Login: Registered LLBR Program Participants are to login to the Activities platform (noted in the table below) or link into your Community’s quarterly e-mail invitation, which takes you to the current quarter’s Activity schedule.

 

Community Events are featured at the Community website (noted below). Each Participant is required to complete at least one (1) Community Event throughout the Entry Period. Quarterly Activities must be completed in each quarter (see Activity Chart below) to be entered into each of the quarter’s random winner (“Winner(s)”) drawings. There are no limits on how many quarters a Participant can be a Winner. However, in order to be eligible to receive any quarter’s prize a Winner must show that one (1) Community Event was completed in either the current specified Activity Quarter Entry Period or in a prior Activity Quarter Entry Period.

Sponsor is not responsible for cancellation or preemption of an Event or for other circumstances that make it impractical or impossible to fulfill the elements of the Event.

The Sponsor and its website’s database clock will be the official timekeeper for Entries. For purposes of this Promotion, an Entry is “received” when the website’s servers record the Entry information upon clicking the final “Submit” button. Proof of sending does not constitute proof of actual receipt of an Entry.

 

2.) Mail-In Alternate Method of Entry (“AMOE”) is available only to eligible individuals (refer to Eligibility Section I) who choose not to participate in the Activity or Event obligation, may enter the Sweepstakes by legibly hand printing on a 3” x 5” card or paper, in black or blue ink, the title of the promotion, the Community name where you reside, first name, last name, email address (if available), mailing address, city, state, zip, phone number with area code first and date of birth. IMPORTANT:  On the outer mailing envelope in the lower left hand corner you must print the Community where you reside and the date of the quarter’s Activity Period Start and End date. Mail the 3" x 5" card or paper in a hand printed first class stamped #10 business envelope to: Compliance / BZP Live Longer, Better Rewards Program Sweepstakes, PO Box 468, Stonington, CT 06378. Return address on envelope must also be hand-printed with sender’s full name and address. Commercial printed return address labels will void the entry. One (1) AMOE may be submitted in each quarter as per the above Quarterly Activities Entry and Winner Drawing Schedule. By submitting an AMOE in any one of the four quarters, a Participant receives one (1) entry (“Entry”) into that quarter’s random drawing. Refer to the above Quarterly Activities Entry and Winner Drawing Schedule (“Quarterly Schedule”) for AMOE mail-in timelines. To be eligible for a Drawing, your mailed Entry must be received no later than 5 business days after the date you have indicated on your mailing envelope. No more than one (1) mail-in entry is allowed or entered into the drawing during each Quarter’s Entry Period. Over limit mail-in entries will be discarded. Sponsor reserves the right to disqualify any person that submits more than maximum allowed entries. Sponsor will not use personal information obtained through your mail-in entry, except in connection with this Sweepstakes. All Entries become property of the Sponsor and will not be acknowledged or returned. Entries that are forged or altered or generated by a macro, bot, or other automated means will not be accepted and will be void.

IV. WINNER SELECTION, NOTIFICATION AND ODDS: From January 1, 2022 through December 31, 2022, a separate, random quarterly drawing from among all eligible Entries received by the last day of each quarter will be conducted on or about the 7th day of the month after the applicable quarter’s Entry End Date, by the Sweepstakes Administrator, an independent judging organization, whose decisions are final and binding in all matters relating to the selection of the Prize winners (“Winner(s)”) of this Sweepstakes. Twenty (20) Winners from a participating Community will be randomly selected each quarter. Winners from any quarter are eligible to win prizes in other quarters. Non-winning Participants are eligible in subsequent quarterly drawings by logging into their Account and completing that quarter’s health related Activity and one (1) Community Event. Winners will be notified by e-mail that he/she may have won a prize, on or about ten (10) days following the applicable Winner Drawing Date. Winner is not required to respond to the prize information e-mail. If a potential Winner’s e-mail is returned as undeliverable, such potential Winner will be disqualified and an alternate potential Winner may be selected at Sponsor’s sole discretion. Odds of winning a prize will depend on the total number of eligible Submissions and Entries received at the time of the quarterly drawings. 

In the event of a dispute regarding who submitted an entry, the entry will be deemed submitted by the authorized account holder of the e-mail account specified in the entry. “Authorized account holder” is defined as the natural person who is assigned to an e-mail address by an Internet access provider, online service provider, or other organization (e.g., business, educational institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address. Winner(s) may be required to show proof of being the registered account holder. In the event a dispute regarding the identity of the individual who actually submitted an entry cannot be resolved to Sponsors’ satisfaction, the affected entry will be deemed ineligible. The Release Entities are not responsible for and shall not be liable for late, lost, damaged, intercepted, misdirected, or unsuccessful efforts to notify the potential Winner. The Released Entities (as defined in Section I) are not responsible for electronic communications that are undeliverable as a result of any form of active or passive filtering of any kind, or insufficient space in Entrant’s e-mail account to receive e-mail messages.

V. PRIZES (“Prize(s)”): Quarterly, twenty (20) First Prize Winners, eighty (80) in total Community Winners are to receive a $50.00 valued item from the BZP online Sharecare Rewards Store, located at https://www.drewandrogers.com/sharecarebz/. Prizes must be redeemed within sixty (60) days of a Winner’s notification, or the Prize will be forfeited. The value of the Prize is subject to price fluctuations in the consumer marketplace based on, among other things, any gap in time between the date the ARV is estimated for purposes of the Official Rules and Terms and Conditions and the date the Prize is awarded or redeemed, however in no event will the ARV exceed Fifty dollars ($50.00). Sponsor reserves the right, at its sole discretion, to substitute any Prize with another Prize of equal or greater value in the event that the advertised Prize (or any component thereof) is not available.  All decisions by Sponsor as to Winners and Prizes are final. Total Prize Pool’s Approximate Retail Value (“ARV”) of the participating Community is $4,000.

PRIZES ARE AWARDED "AS IS" AND WITHOUT WARRANTY, GUARANTY OR REPRESENTATION OF ANY KIND, EXPRESS OR IMPLIED.

VI. PRIZE CONDITIONS: Each Winner is required to comply with any and all applicable federal, state and local laws, rules and regulations. Winner is solely responsible for all matters relating to the Prize, including, but not limited to all applicable federal, state and local taxes that become due with respect to the Prize. A Winner may be required to send a copy of their passport, driver’s license or birth certificate as proof of age and residence. Non-compliance may result in disqualification. If a Prize or Prize notification is returned as unclaimed or undeliverable to a potential Winner, if potential Winner cannot be reached within three (3) calendar days from the first notification attempt, as set forth in Rule IV, or if potential Winner fails to return requisite documents within the specified time period, or if a potential Winner is ineligible or not in compliance with these Official Rules, then such person shall be disqualified and, an alternate Winner may be selected for the Prize at issue. The Prize Winner is subject to verification by Sponsor. The Prizes are not exchangeable, transferable, or redeemable for cash, except in Sponsor's sole discretion.

VII. MODIFICATION: In no event will Sponsor be obligated to award more Prizes than the number of Prizes stated in these Official Rules.  If for any reason Sponsor determines, in its sole discretion, that the Program is no longer capable of running as intended by these Official Rules for any reason whatsoever, Sponsor reserves the right to cancel, terminate, suspend, postpone, delay or modify the Program, in whole or in part, including, but not limited to, any occurrences described in Rule X, or any other causes which may corrupt or impair the integrity, security, fairness or proper conduct of the Promotion and Program.  If Sponsor determines that more Prize claims are submitted or more Prize notices are distributed than the number of Prizes offered in these Official Rules, Sponsor shall immediately suspend all drawings and Prize awards, and Sponsor reserves the right to award any remaining Prizes (up to the total set forth in these Official Rules) in a manner deemed fair and equitable by Sponsor. For example, Sponsor may select a Winner in a random drawing from among the eligible Entries received up to the time of the impairment or conducted a random drawing among all Prize claimants, and neither Sponsor, the Sweepstakes Administrator nor any of the Released Entities shall have any further liability to any Participant in connection with the Promotion.
 
VIII. INTERNET LIMITATIONS OF LIABILITY: If for any reason this Promotion is not capable of running as planned for any reason, including, without limitation, due to infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes beyond the control of the Sponsor which corrupt or affect the administration, security, fairness, integrity or proper conduct of this Sweepstakes, the Sponsor reserves the right, in its sole discretion, to disqualify any individual who tampers with the entry process, and to cancel, terminate, modify or suspend the Promotion, in whole or in part, at any time, without notice and award the Prizes using all non-suspect eligible Entries received as of or after (if applicable) this termination or suspension date. Any person who supplies false information, obtains Entries by fraudulent means or is otherwise determined to be in violation of these Official Rules in an attempt to obtain any prize will forfeit any prize won and may be prosecuted to the full extent of the law.  CAUTION: ANY ATTEMPT TO DAMAGE ANY WEB SITE OR UNDERMINE THE LEGITIMATE OPERATION OF THE CAMPAIGN IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES OR OTHER REMEDIES FROM ANY SUCH PERSON(S) RESPONSIBLE FOR THE ATTEMPT TO THE FULLEST EXTENT PERMITTED BY LAW. In the event of a dispute regarding the identity of an individual who actually submitted an Online Entry, the Entry will be deemed submitted by the authorized account holder of the e-mail account. "Authorized Account holder" is defined as the natural person who is assigned to an e-mail account by an Internet access provider, Online service provider or other organization (e.g., business, educational, institution, etc.) that is responsible for assigning email addresses for the domain associated with the submitted e-mail address.

IX. RELEASES: All Participants, as a condition of participation in this Promotion, agree to indemnify, release and hold harmless Sponsor, Sponsor Partners and Release Entities from and against any and all liability, claims, damages, injuries or actions of any kind whatsoever for injuries, damages, or losses to persons or property which may be sustained in connection with: (a) accessing the Sponsor website, (b) participating in any aspect of the Promotion, including, without limitation, while preparing for, participating in and/or traveling to or from any Prize or Promotion related Activity, including, without limitation, any injury, damage, death, loss or accident to person or property, (c) the receipt, ownership, possession, use or misuse of any Prize awarded, (d) viruses or any downloading or other problems with the Promotion application, or (e) any typographical or other error in these Official Rules or the announcement of offering of any Prize. Sponsor makes no representations or warranties of any kind concerning the appearance, safety or performance of any Prize, except for any express manufacturer's warranty as may be included with the Prize. The Winner bears all risk of loss or damage to his/her respective Prize after it has been delivered.

X. PUBLICITY: Subject to applicable law, by participating in a Promotion, Winner(s) irrevocably grant the Released Entities, and each of their licensees, successors, assigns and sub-licensees, the right and permission to use their name, voice, likeness and/or biographical material for advertising, promotional and/or internal or external publicity purposes in connection with the Promotion in which Winner has participated, in all forms of media and by all manners (now and hereafter known), and on and in connection with related products, services, advertising and promotional materials (now known or hereafter developed), worldwide, in perpetuity, without any obligation, notice or consideration except for the awarding of the Prize(s) to the Winner(s).

XI. USE OF DATA AND PRIVACY: By participating in the Sweepstakes, you hereby authorize Sponsor the right to collect, use, process, transfer and store all data provided by you, including without limitation, personal data, and share with such party(ies) as Sponsor deems appropriate, for purposes of administration of the Sweepstakes Promotion. All information submitted to the Sponsor as part of this Promotion will be treated in accordance with the Sponsor’s privacy policy, available at https://www.sharecare.com/terms/privacypolicy and only be used by the Sponsor to administer the Program and, to provide the Entrants with information regarding upcoming promotions and/or events from the Sponsor, or by a third party, such as a vendor involved in, or providing services in connection with a Promotion. Participants have the right to access, withdraw, and correct their personal data. Directions on procedure for administrative access to personal data are provided in the privacy policy. 

XII. GOVERNING LAW/DISPUTES: By participating, Participants agree to be bound by the Official Rules, Sponsor's Web Site Terms of Use (if any and if applicable to Participant's method of participating), and the decisions of the Sponsor and Administrator, which are final and binding in all respects. The Sweepstakes and these Official Rules shall be construed and governed in accordance with the laws of the State of Georgia.  All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules or the rights and obligations of Participants, Sponsor or the Release Entities in connection with the Sweepstakes will be governed by and construed in accordance with the internal laws of the State of Georgia, without giving effect to any choice of law or conflict of law rules or provisions that would cause the application of any other states laws.  Except where prohibited, as a condition of participating in this Promotion, the parties agree that any and all disputes which cannot be resolved between the parties, claims and causes of action arising out of or connected with this Sweepstakes, or any prizes awarded, or the determination of winners shall be resolved individually, without resort to any form of class action exclusively by arbitration pursuant to the commercial arbitration rules of the American Arbitration Association, then effective. Further, in any such dispute, under no circumstances will Participant be permitted to obtain awards for, and hereby waives all rights to claim punitive, incidental or consequential damages, or any other damages, including attorneys' fees, other than Participant's actual out-of-pocket expenses (e.g., costs associated with entering this Sweepstakes), and Participant further waives all rights to have damages multiplied or increased. The arbitration shall be conducted in the State of Georgia, in the City of Atlanta, County of Fulton, and judgment on the arbitration award may be entered into any court having jurisdiction thereof.  The arbitrator will apply the substantive laws of the state of Georgia, will issue a written decision and will have the power to award any legal remedies except as limited by these Official Rules. The parties will split the arbitrator’s fee; provided, however, if applicable law requires Sponsor to pay the arbitrator’s fee in order for the arbitration provision to be enforceable, Sponsor shall have the discretion to elect to pay such fees and proceed to arbitration.  Participant irrevocably waives any right to bring or join in any class action regarding the Sweepstakes. Judgment upon the arbitration award may be entered in any court having jurisdiction thereof.  This arbitration provision shall be deemed to be self-executing, and in the event that any party fails to appear to any properly noticed arbitration proceeding, an award may be entered against such party notwithstanding said failure to appear.
 
XIII. WINNERS LIST/OFFICIAL SWEEPSTAKES RULES REQUEST: For the names and city/state of Prizewinners, or a copy of these Official Rules, send a request specifying a copy of the Winners' List or Official Rules along with a self-addressed, stamped envelope received by February 18, 2023 mail to: Compliance/ BZP Winners List, PO Box 468, Stonington, CT 06378.

XV. ADMINISTRATOR: Compliance Sweepstakes Services LLC, PO Box 469, Stonington, CT 06378 (the “Sweepstakes Administrator”).


XVI.  MISCELLANEOUS: The names of individuals, groups, companies, products and services mentioned herein, and any corresponding likenesses, logos and images thereof reproduced herein, have been used for identification purposes only and may be the copyrighted properties and trademarks of their respective owners. The mention of any individual, group or company, or the inclusion of a product or service as a prize, does not imply any association with or endorsement by such individual, group or company or the manufacturer or distributor of such product or service and, except as otherwise indicated, no association or endorsement is intended or should be inferred. The invalidity or unenforceability of any provision of these Official Rules or the Prize Acceptance Release will not affect the validity or enforceability of any other provision.  In the event that any provision of the Official Rules or the Prize Acceptance Release is determined to be invalid or otherwise unenforceable or illegal, the other provisions will remain in effect and will be construed in accordance with their terms as if the invalid or illegal provision were not contained herein.  Sponsor’s failure to enforce any term of these Official Rules will not constitute a waiver of that provision.  For Participant, to the extent there is a conflict between these Official Rules and any terms of use or other agreement posted on the Sponsor's Website these Official Rules control.
